Report Your Injury


After sustaining a workplace injury, the most important step is to report the injury to your employer. Your employer will then report your claim to United Heartland.

Why is it important?

At United Heartland, we work with our customers to help keep their workers safe on the job. When a workplace injury occurs, we want you to receive the medical care and the workers’ compensation benefits entitled to you quickly and efficiently, so you can return to work.

How do I do that?

Report your injury to your immediate supervisor or human resources department following your work injury. At that time, they will assist you in filling out a First Report of Injury Form, which will then be sent to United Heartland.

When should I do it?

As soon as possible.



Treat Your Injury


At United Heartland, we are committed to ensuring that your care is appropriate, prompt and helpful for your return to work.

Why is it important?

State rules and regulations determine whether you are able to see your own doctor for treatment or change your treating physician.

How do I do that?

You should speak to your United Heartland claim representative or nurse case manager to discuss whether treatment with your personal doctor will be approved prior to making any change in treatment.



Keep Us Updated


It is very important to remain in contact with your employer and your United Heartland claim representative to let them know how your care and treatment are progressing. You must also actively participate in recommended medical treatment plans. The goal of your medical treatment is to recover quickly and return to work.

Why is it important?

Keeping your claim representative current on your status is crucial to ensuring you receive your benefits timely and accurately.

How do I do that?

Within 24 hours of receipt of your First Report of Injury, your claim representative will call you to discuss your claim. You will be provided their contact information, including phone number, so you can provide updates as needed.

When should I do it?

The frequency of contact with your claim representative depends on the severity of your work injury. If you have been recommended for conservative treatment, such as physical therapy, we recommend following up with your claim representative every two weeks. Additionally, you should provide your claim representative with updates following re-evaluation appointments with the physician so they can document your treatment plan, any changes in restrictions or referrals to a specialty physician.
